Immunization Record (PPD, Hepatitis B, MMR, Tetanus).
Parental consent forms for fingerprinting and PPD if less than 18 years of age.
Criminal Background check form.
Original Work Permit if less than 18 years of age.
Professional License.
Current CPR card.
